3. Complete Form DS-2029 Application for Consular Report of Birth Abroad (CRBA). Official website link. (Their link is broken at the time of post)
(TIP) – MAKE SURE TO ACCURATELY DOCUMENT YOUR TRAVEL DATES IN ORDER AS REFLECTED IN ALL YOUR PASSPORTS (YOUR CURRENT PASSPORT AND ALL THE EXPIRED ONES) INACCURACIES AT THE INTERVIEW CAN LEAD TO A RESCHEDULING OF INTERVIEW.
** DO NOT SIGN DS-5507 – AGAIN DO NOT SIGN DS-5507 UNTIL YOU ARE TOLD BY THE INTERVIEWING OFFICER.
** IF THE US CITIZEN PARENT WILL NOT ATTEND THE 2ND INTERVIEW THE DS-5507 MUST BE SIGNED AND NOTARIZED BY A US NOTARY.
